Job summary

Single Homeless Project is seeking an Outreach Housing Pathway Coordinator to help rough sleepers access and sustain accommodation within Camden’s integrated hub. You will oversee housing journeys, identify blockers and work with outreach teams, hostels and services to move plans forward with urgency and accountability.

You will champion trauma-informed practice, build partnerships and maintain strong engagement to reduce the risk of returning to street life.

Qualifications

• Understand trauma, exclusion, unmet needs and systems that have not worked for them.
• Able to spot delays, exclusions or gaps risking routes away from the street.
• Confident with complexity, risk, need and accountability while keeping safety and dignity central.
• Build relationships, communicate clearly, and follow through on actions to move plans forward.
• Trauma-informed approach, emotional resilience and calm under pressure.

Responsibilities

• Hold oversight of where people are in their housing journey and identify blockers.
• Collaborate with outreach teams, Navigators, hostels, pathway providers and statutory services to keep plans moving.
• Engage assertively to reduce delays, exclusions and unmet needs impacting housing outcomes.
• Work within Camden’s No Wrong Door model to ensure joined-up, trauma-informed responses.
