Job summary

Care & Repair Monmouthshire and Torfaen is seeking a highly organised, enthusiastic H2H Caseworker to support older people to return home from hospital and live independently in their own homes. The role involves visiting patients in hospital wards and in their own homes to assess needs and provide practical assistance.

You will help improve home safety with adaptations, repairs and services while offering advice on welfare benefits, housing grants and heating funding.

Qualifications

• Knowledge of welfare benefits, older people's rights and entitlements.
• Experience in housing related service or health/social care setting is desirable.
• Full driving licence and access to a car are essential and a DBS check is required.

Hours: 37 hours a week (Monday to Friday)

About the role

The post holder will work closely with health and social care professionals as well as third sector organisations to support older people to return home from hospital and remain living independent in their own homes.

This will include visiting older people on hospital wards, along with following up in their own homes. The H2H Caseworker will help to improve the patient's home environment by providing a range of adaptations, repairs and home safety interventions that will help prevent the risk of falls and accidents in the home and improve their long-term independence and well-being.

A key part of the role will also include providing advice and practical support in relation to accessing welfare benefits, housing grant-aid, heating grants and charitable funding as appropriate. It is desirable that the post holder will have a good knowledge of welfare benefits, older people's rights and entitlements and will have worked within a housing related service or health/social care setting. Comprehensive training will be provided.

A full driving licence and access to a car is essential and the post is subject to a DBS check paid for by the Agency.
