Job summary

Warwickshire County Council is seeking a Level 2 Social Worker to join the Hospital Social Care Team. You will manage a caseload, undertake robust assessments, and support care and discharge planning for older adults across Warwickshire’s hospital sites.

You will work five days over seven, including weekends on a rolling rota, with supervision and access to training. A dynamic, strength‑based approach is encouraged in a fast-paced hospital environment.

Qualifications

• Professional Social Work qualification and HCPC registration are essential.
• Able to work 5 days over 7 to support streamlined service and timely discharges.
• Experience in hospital social work and strong IT literacy are desirable.

Responsibilities

• Provide a social work service to a high professional standard in a hospital setting.
• Manage a caseload and travel across North and South Warwickshire as required.
• Undertake robust assessments and care and support planning with carers and families.
