Job summary

Warwickshire County Council's Hospital Social Care Team is expanding to support older adults in a fast-paced hospital setting. You will manage caseloads, undertake robust assessments and care planning, and collaborate with health partners across Warwickshire.

Based at George Eliot Hospital in Nuneaton, you will work five days per week with some weekend work; a driver’s licence is essential for post-discharge reviews in care homes. Training and supervision are provided by the council.

Qualifications

• Must have a professional Social Work qualification and be HCPC registered.
• Be able to work 5 days over 7 days, with one weekend in a month on a 4-week rota.

Responsibilities

• Provide a social work service to a highly professional standard and manage a caseload.
• Travel across the locality of North and South Warwickshire as required.
• Undertake robust assessments and care and support planning.
• Communicate with customers, communities, health partners and agencies.
• Participate in team meetings and supervision led by the Team Manager.
