Job summary

Community Care Live are seeking a qualified Social Worker to join the Basingstoke Hospital Social Work Team. You will assess needs, manage a complex caseload and support colleagues in delivering social care for adults leaving hospital.

You must be registered with Social Work England, have experience with adults in social care, and apply a strengths-based approach while upholding statutory duties and values of compassion and respect.

Qualifications

• Social Work qualification and SWE registration.
• Experience of working with adults within a social care setting.
• Strengths-based approach in practice.
• Understanding of social work issues, legal frameworks and statutory duties.
• Compassion, empathy and respect in values and behaviours.

Responsibilities

• Conduct comprehensive assessments of individuals' needs and available support.
• Manage a complex caseload whilst supervising Case Workers and Senior Case Workers and providing practice advice on complex cases.
• Triaging safeguarding referrals and making informed decisions.
• Building positive and professional relationships.
