Job summary

The British Red Cross Society is seeking a dedicated Support Worker to join the 7-day hospital discharge service in Sandwell. You will collaborate with hospital teams and partner organisations to help people return home safely after a hospital stay and maintain independence.

You will be part of a supportive team and operate in a hands-on, community-based role across Sandwell, with a clear shift pattern and opportunities for flexible working.

Responsibilities

• Respond to referrals and complete welfare calls and home visits.
• Attend hospital wards and discharge units to support patients.
• Provide practical and emotional support to aid recovery and independence.
• Assess needs, identify safeguarding concerns, and accurately record case notes.
• Signpost individuals to appropriate services and community resources.
