South Tyneside Council is seeking a dedicated Social Worker to join the Adult Social Care Hospital Discharge Team. You will manage a varied caseload, conducting needs assessments and co-producing care plans to maximise independence and safeguard wellbeing.

You will collaborate with individuals, carers, and multi-agency partners, promoting direct payments and community resources while maintaining high standards of records and timely reviews.

Qualifications

• Recognised Social Work qualification and active registration with Social Work England.
• Ability to manage complex caseloads and conduct safeguarding investigations.
• Excellent communication and report writing standards.

Responsabilités

• Undertake needs assessments and co-produce care plans tailored to outcomes and resources.
• Promote person-centred support and use of direct payments where appropriate.
• Carry out safeguarding investigations in line with local procedures when required.
• Regularly review support packages to ensure cost-effective care for the required duration.
• Build local networks and liaise with health partners and other services.
• Produce high-quality reports and represent the service in court when required.
• Engage in supervision and stay updated with legislation and best practices.
