Derbyshire County Council invites applications for a permanent Social Worker to join the Hospital Discharge Team at the Royal Derby Hospital. You will facilitate safe, sustainable transfers of care for people leaving hospital, with flexible placements across Derby hospitals and the care transfer hub as needed.

The role requires transferable skills, relationship-building with health partners, and a person-centred approach within timescales of the Care Act.

Qualifications

• Registered social worker with current HCPC registration.
• Experience in hospital discharge or care transfer desirable.
• Ability to work in a multi-disciplinary environment.

Responsabilités

• Facilitate safe, timely transfers of care for patients leaving hospital.
• Collaborate with Health Partners and care providers to maintain smooth patient flow.
• Adapt to service changes and support flexible working patterns.
