Job summary

Carrington Blake Recruitment is supporting Hammersmith & Fulham Council in London to recruit an experienced Social Worker for the Hospital Discharge Service at Charing Cross Hospital. You will assess adults’ needs, develop discharge plans and work with a multidisciplinary team to promote safe, timely hospital departures and ongoing wellbeing.

Qualifications

• Recognised Social Work qualification (Degree/DipSW/CQSW).
• Current registration with Social Work England.
• Minimum three years’ post-qualification experience in a statutory setting.
• Experience with adults with complex needs and safeguarding inquiries.
• Knowledge of NHS Continuing Healthcare and hospital discharge processes.

Responsibilities

• Assess adults’ care and support needs and develop discharge plans.
• Collaborate with hospital staff, families and care providers for safe discharges.
• Develop strength-based plans promoting independence and wellbeing.
• Arrange care packages, reablement services and community support.
• Undertake safeguarding enquiries and assess risks.
• Conduct mental capacity assessments and best-interest decisions.
• Provide guidance on NHS Continuing Healthcare and Direct Payments.
• Apply Care Act 2014 and related legislation to assessments and care planning.
• Maintain accurate case records and communicate recommendations clearly.
