Job summary

Sanctuary Personnel is seeking an Adults Social Worker – Hospital Team for Powys, Wales, delivering safe discharges for residents aged 65+. This ongoing, full-/part-time role supports the hospital discharge process and collaborates with social care teams to ensure smooth transitions back home.

The role emphasizes statutory responsibilities, flexible/agile working, and the opportunity to contribute to high-quality care in the community while working within a supportive Powys Council framework.

Responsibilities

• Collaborate with Powys residents aged 65 and over to enable their safe discharge from acute hospital settings, adhering to Welsh Government hospital discharge service requirements and using discharge to recover and assess guidance.
• Identify the most suitable discharge pathway for each individual, promoting a What Matters strengths-based approach to enhance their independence.
• Communicate effectively with professionals, individuals, families, and carers to agree on the correct discharge pathway and address barriers preventing timely discharge.
• Work closely with system partners and the Hospital Social Work Team to resolve these barriers, ensuring a smooth transition for Powys residents back to their homes.
• Fulfil statutory obligations under the Mental Capacity Act 2005, the Mental Health Act 1983, the Mental Health Wales Measure 2010, and the Social Services and Wellbeing Wales Act 2014.
