Job summary

Hoop Recruitment Ltd is actively seeking a Social Worker for Powys County Council to focus on hospital discharge planning for 65+ residents. The role emphasizes safeguarding, independence, and timely transfers from hospital settings.

The successful candidate must hold a Social Work qualification, be registered with Social Care Wales or Social Work England, and have an enhanced DBS. Driving license and references are required.

• 3 years of referencing.

Responsibilities

• Identify discharge pathways using a strengths-based approach to safeguard and promote independence.
• Support residents 65+ in Wales to safely transfer from acute hospital settings.
• Liaise with professionals, the individual, family and carers to determine appropriate discharge pathways and address barriers.

Hospital - Social Worker

Pay rate - up to £37

Key Responsibilities

• To identify the most appropriate discharge pathway for a person which supports a 'What Matters' strengths-based approach to safeguard and promote their independence.


• To work alongside Powys residents aged 65 + to facilitate their safe and timely transfer from acute hospital settings in line with Welsh Government hospital discharge service requirements and utilising discharge to recover and assess guidance.


• To liaise with professionals, the individual, their family, and carers to determine a person's correct discharge pathway and to raise any obstacles which impact upon timely discharge.
