Job summary

Sanctuary Personnel is seeking an experienced Adults Social Worker to join the Hospital Discharge Team in South Shields, Tyne and Wear. You will assess, plan and support safe discharges in partnership with NHS teams and community providers.

Applicants should hold a Social Work degree and be registered with Social Work England. This role offers ongoing work with full-time or part-time options and a supportive, collaborative environment.

Qualifications

• Degree in Social Work or equivalent qualification.
• Evidence of continuing professional development relevant to Adult Social Care and Hospital Discharge services.

Responsibilities

• Undertake Care Act 2014 assessments, reviews and support planning for adults in hospital and discharge pathways.
• Facilitate safe and timely hospital discharges in partnership with NHS multidisciplinary teams and community providers.
• Complete Mental Capacity Act assessments and support best-interest decision-making where required.
• Manage a complex caseload including older people, adults with physical disabilities, frailty and complex social care needs.
• Participate in multidisciplinary teams, discharge planning discussions and safeguarding enquiries.

What you will do:

Qualifications:

• Degree in Social Work, DipSW, CQSW or an equivalent recognised Social Work qualification.
• Evidence of continuing professional development relevant to Adult Social Care and Hospital Discharge services.

Registration and Compliance:

• Current registration with Social Work England.

Experience:

• Post-qualification experience within Adult Social Care, Hospital Teams, Community Teams, or Integrated Care Services.
• Experience undertaking Care Act assessments and managing complex adult social care cases.
• Experience working alongside NHS professionals within acute hospital environments.
