Job summary

Northumberland County Council is seeking a Social Worker for the Home Safe Hospital Discharge Service at Wansbeck General Hospital in Northumberland. The role supports safe hospital discharges, with a balance of autonomy and supervision, focusing on comprehensive psychosocial assessments and care planning.

You will work with individuals and carers, complete essential assessments and coordinate with external services to meet care needs, while upholding Care Act duties.

Qualifications

• Degree in Social Work and registered with Social Work England.
• Successful completion of the UK Skills for Care ASYE programme.
• Must hold a valid UK driving licence and be able to use a car for work.

Responsibilities

• Assess complex psychosocial needs and develop care plans for safe hospital discharges.
• Complete Urgent Needs Assessments and Mental Capacity Assessments; liaise with external services.
• Manage caseload with supervision and support from the Team Manager.
