Leicestershire Partnership NHS Trust is seeking a part-time Discharge Nurse (22.5 hours over 3 days) at the Evington Centre, Leicester General Hospital. You will oversee the 117 referral process for inpatients and help ensure safe discharge with the multidisciplinary team.
The role offers a supportive, learning-focused environment with induction, regular supervision, and opportunities for career progression within the Patient Flow Team.
Responsabilités
- Complete the 117 referral process for inpatients at the Evington Centre.
- Assist our Trusted Assessors to ensure safe discharge for all inpatients.
- Work within the multi-disciplinary team to determine appropriate community support.
