Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is looking for a Healthcare Support Worker to deliver high-quality care in patients' homes across Southampton, Eastleigh, Lymington and Waterside. This fast-paced role involves basic health observations, rehabilitation support, personal care, mobility help and emotional wellbeing to promote independence.

You will join a supportive MDT with induction and ongoing development.

Qualifications

• NVQ Level 3 (or equivalent experience) or strong care experience.
• Experience in a care, hospital, or community setting.
• Ability to work independently and manage a varied workload.
• Compassionate, reliable, and good communication skills.

Responsibilities

• Deliver delegated clinical, rehabilitation, and personal care in patients' homes
• Undertake observations and support assessments
• Support people with frailty, long-term conditions, mobility needs, and end-of-life care
• Work as a lone worker, with robust clinical support
• Accurately record care using electronic systems
