Job summary

Longfield Hospice is seeking an experienced Registered Nurse to join our Hospice at Home team, delivering compassionate end-of-life care in patients' homes across Gloucestershire. You will provide clinical leadership, support the MDT, and ensure timely, safe care with family involvement.

Responsibilities include line managing HCAs and RNs, coordinating referrals, and upholding governance and education standards to improve service quality while reducing unnecessary hospital admissions.

Qualifications

• Registered Nurse with NMC PIN and current UK registration.
• Experience in community or palliative care settings.
• Evidence of supervision and mentoring of staff.

Responsibilities

• Lead and support Hospice at Home team to deliver high-quality care in patients' homes.
• Line manage HCAs and RNs, providing supervision and performance reviews.
• Coordinate referrals and liaise with GPs, district nurses and other partners.
• Provide clinical leadership, assessment and guidance to staff, patients and families.
• Contribute to governance, policies and education to improve service quality.
• Develop and monitor care plans to prevent unnecessary hospital admissions.
