Job summary

Beaumond House Hospice Care in Mansfield is seeking an experienced Hospice at Home Care Assistant. You will provide hands-on personal care in patients' homes under RN supervision, supporting families and following care plans.

After training, you may visit independently to deliver delegated care with clear escalation routes. Roles include medicines support, care after death responsibilities, and close communication with patients and families.

Qualifications

• Flexible, attentive and capable of building rapport with patients and families.
• Clear spoken and written English for safe care and documentation.
• Willingness to complete medicines support and food hygiene training.
• Ability to work as part of a team and respect all individuals.

Responsibilities

• Provide personal care per plan under RN supervision, supporting dignity and privacy.
• After competency sign-off, make independent visits delivering delegated care.
• Support medicines handling under RN direction and training.
• Communicate sensitively with patients and families, adapting to needs.
• Provide care after death, respecting cultural and spiritual needs.
• Verify expected death in the community under hospice policy after training.
• Prepare and serve simple meals and drinks per food hygiene rules.
• Liaise with funeral directors for practical next steps after death.
• Keep accurate records at point of care and share with RN.
