Job summary

NHS Jobs is seeking an experienced navigator of care to coordinate services for patients who are homeless, working across health and social care to connect patients with the right teams. The role involves liaising with partner agencies, reducing unnecessary ED visits through improved access to housing, substance use recovery, and primary care services.

Responsibilities include home visits, multidisciplinary collaboration, accurate record keeping in SystmOne, and contributing to pilot reports.

Qualifications

• Experience in patient coordination and navigation of care
• Experience liaising with health and social care professionals
• Experience collecting data for audits and QOF/IIF monitoring

Responsibilities

• Coordinate and navigate care for patients who are homeless across health and social care systems
• Engage with agencies to improve patient engagement with healthcare and social priorities
• Support patients accessing Emergency Department at LRI and ensure timely access to assessments and treatment
