Job summary

County Durham & Darlington NHS Foundation Trust is seeking a Community Healthcare Assistant to join our community nursing team. You will provide nursing care to patients in their own homes, including insulin administration and wound care, often working alone with a care plan and risk-aware approach.

The role requires good communication, teamwork and the ability to observe and report clinical changes. Full training will be provided and you will travel across trust sites as needed.

Qualifications

• NVQ Level 3 / Diploma in Health & Social Care or equivalent.
• GCSE English and Mathematics.
• Knowledge of Microsoft Packages (Office).
• Understanding of County Durham and Darlington NHS Foundation Trust, its culture and NHS standing.

Responsibilities

• Provide nursing care to patients in their home or living environment.
• Carry out basic wound care and clinical observations.
• Administer insulin and manage simple clinical tasks as required.
• Record observations and work to a care plan with minimal supervision.
