Job summary

Cornwall Partnership NHS Foundation Trust is seeking a Home First East Team Assessor to join our multidisciplinary community team. You will triage referrals, assess patients in their homes or care settings, and plan evidence-based treatments to help people stay independent and avoid unnecessary hospital stays.

You will work with nursing staff, therapists and admin colleagues to deliver high-quality, person-centred care across Cornwall.

Qualifications

• Degree or diploma in Occupational Therapy or Physiotherapy.
• HCPC registered, NMC Registered.
• Substantial post registration clinical experience in a similar role.
• Extensive experience of working within a multidisciplinary team.
• Postgraduate CPD to support practice; degree equivalent level.

Responsibilities

• Triaging referrals and undertaking holistic assessments.
• Developing and delivering evidence-based treatment plans for adults.
• Working in patients' homes, residential and nursing homes to support independence and avoid hospital admissions.
• Collaborating with community and acute services, Adult Social Care and primary care to ensure seamless, person-centred care.
• Contribute to high-quality, compassionate community-based care.
