The Home First East Team covers the Caradon area in East Cornwall and delivers a responsive, multidisciplinary approach to supporting intermediate care needs. Qualifications

• Degree or diploma in Occupational Therapy or Physiotherapy.
• HCPC or NMC registration.
• Post-registration clinical experience in a similar community role.
• Experience in a multidisciplinary team.
• Evidence of CPD and postgraduate courses.
• Clinical experience in community and inpatient settings.
• Leadership in diverse and complex situations.
• Ability to manage clinical risk in lone-working.
• Safeguarding awareness and reporting.
• Excellent verbal and written communication across boundaries.
• Ability to communicate sensitive information appropriately.
• Understanding of evidence-based care and clinical effectiveness.
• IT literate.

Responsibilities

• Triage referrals and undertake holistic assessments.
• Develop and deliver evidence-based treatment plans for adults.
• Work in patients' homes, residential and nursing homes.
• Collaborate with community and acute services, social care, primary care and voluntary sector.
• Promote independence and avoid unnecessary hospital admissions.
• Contribute to discharge planning and coordination across teams.
