Job summary

Leeds Community Healthcare NHS Trust is seeking a Band 5 Discharge Coordinator to join the Short Term Care Beds Service on a permanent part-time basis (15 hours per week). You will help coordinate safe, timely discharges and support people to recover at home with community teams.

Working in a Home First approach, you will liaise with patients, families and the wider multidisciplinary team to remove barriers to discharge and promote patient independence and wellbeing in the community setting.

Qualifications

• Band 5 Discharge Coordinator role within NHS Banding framework.
• Strong communication and organisational skills are essential.
• Proactive approach to remove barriers and coordinate safe discharges.

Responsibilities

• Coordinate discharge plans for patients
• Support patient flow and bed management
