Job summary

NHS Highland invites applications for an Enablement Worker to join the Care at Home and Enablement Team covering East Ross. The role focuses on helping people remain independent in their own homes and preventing unnecessary hospital admissions.

You'll work as part of a multi-disciplinary team with a senior enablement worker overseeing the post, delivering person-centred care, supporting timely discharges and regular client assessments.

Qualifications

• Must be able to work in a multi-disciplinary team with a person-centred approach.
• Applicants should have or be willing to work towards SVQ 2.
• Good communication and self-motivation are essential for this role.

Responsibilities

• Support people in their own homes to maintain independence.
• Promote independence and prevent unnecessary hospital admissions.
• Assist with timely hospital discharges and ongoing assessments at home.
