Job summary

NHS Highland’s Learning Disability Service is seeking Outreach Social Care Support Workers to support vulnerable adults in their own homes and communities in Skye & Lochalsh. The role involves lone working, shifts including nights/weekends, and travel within the local area.

We value compassionate, motivated individuals with good communication skills and the ability to work both independently and as part of a wider health and social care team. SVQ Level 3 in Health & Social Care is encouraged.

Are you passionate about supporting vulnerable adults to live fulfilling, independent lives in their own homes and communities?

NHS Highland’s Learning Disability Service is looking for compassionate, motivated Outreach Social Care Support Workers to join our dedicated team in Skye & Lochalsh. This is a rewarding role where no two days are the same. You will work closely with individuals with learning disabilities, physical disabilities, dementia, and older adults - providing person - centred support that promotes dignity, independence and wellbeing. We have 2 full time (36 hour per week) posts available.

About The Role

• Provide high‑quality care and support within individuals’ own tenancies.
• Support people with daily living skills including personal care, meal preparation, medication support, and maintaining a healthy lifestyle.
• Deliver outreach in the community, enabling social engagement, leisure opportunities, and participation.
• Act as a key worker, contributing to care planning, risk assessments, and case reviews.
• Support individuals to achieve reablement goals and develop independence.
• Work independently as well as part of a wider integrated health and social care team.
• Maintain accurate records, follow care plans, and uphold NHS Highland standards.
• This role involves lone working, shift work (including nights/weekends), and travel within the local area.

What We’re Looking For

• Good communication skills (verbal, written, and IT).
• Ability to work independently and as part of a team.
• Understanding of confidentiality and professional boundaries.
• Willingness to learn and undertake training (including SVQ Level 3 in Health & Social Care).
• Ability to manage physically and emotionally demanding tasks.
• Awareness of the needs of vulnerable adults.
• Experience in care, support work, or volunteering is desirable but not essential.
