Job summary

HRM Homecare Services Ltd is seeking a capable Care Co-ordinator to join our South Lanarkshire team. The role focuses on leading frontline staff to deliver high-quality, person-centred care at home, coordinating resources and safeguarding patient independence.

Qualifications

• Experience supervising, coordinating or managing people, services or complex operational activity.
• Strong organisational skills and the ability to manage competing priorities.
• Confident written and verbal communication.
• Good analytical and problem-solving ability.
• Sound judgement and decision-making within the role.
• Ability to stay calm when circumstances change.

Responsibilities

• Develop, review and maintain high-quality, person-led care and support plans and risk assessments.
• Work with individuals and families to understand what matters to them and promote independence.
• Undertake meaningful reviews and recognise changing needs or outcomes.
• Monitor Self-directed Support arrangements, commissioned hours and resources.
• Identify budget/resource variance and advise management on necessary actions.
• Roster and coordinate staff to match staff to individual needs and ensure continuity.
