Job summary

HCRG Care Group in Swale is seeking a Band 5 Physiotherapist to join the Rapid Response and Intermediate Care Team, based at Sheppey Community Hospital. You will rotate across community and inpatient rehabilitation settings, delivering patient-centred care to maximise independence.

You will work within a multidisciplinary team, supported by clinical leads, with NHS Pension, development opportunities and a comprehensive benefits package.

Qualifications

  • Must hold a Degree or Diploma in Physiotherapy.
  • HCPC registration as a Physiotherapist (or awaiting registration).
  • Excellent communication and interpersonal abilities required.

Are you a passionate Physiotherapist looking to develop your skills across a broad range of community and rehabilitation services? We are seeking a motivated Band 5 Physiotherapist to join our Rapid Response and Intermediate Care Team in Swale. This is an exciting rotational opportunity that will enable you to build experience across community rehabilitation, rapid response services and inpatient rehabilitation. Working as part of a supportive multidisciplinary team, you will collaborate closely with Physiotherapists, Occupational Therapists, Nurses, Support Workers and wider healthcare professionals to deliver high-quality, patient-centred care. The service supports patients in their own homes and within our inpatient rehabilitation unit, helping individuals maximise their independence, achieve their rehabilitation goals and avoid unnecessary hospital admissions. The service operates seven days a week between 8am and 8pm and includes weekend and bank holiday working on a rota basis. Support is provided by experienced Clinical Leads for Physiotherapy, Occupational Therapy and Nursing, ensuring, HCRG Care Group is looking for a passionate Band 5 Physiotherapist to join Swales Rapid Response and Intermediate Care Team. The position offers a rotational opportunity to gain experience across community rehabilitation, rapid response services, and inpatient rehabilitation. You will work as part of a supportive multidisciplinary team to provide high-quality, patient-centred care, encouraging independence and preventing unnecessary hospital admissions. The service operates daily between 8am and 8pm, including weekends and bank holidays. Key Responsibilities
  • Undertake six-month rotations across Rapid Response, Intermediate Care and the Inpatient Rehabilitation Unit
  • Assess, plan, implement and evaluate physiotherapy treatment programmes for patients with a variety of complex physical health conditions
  • Complete comprehensive, holistic assessments and develop person-centred rehabilitation goals
  • Deliver evidence-based interventions designed to maximise independence and functional ability
  • Manage your own clinical caseload with support from senior colleagues as required
  • Work collaboratively with patients, carers, families and multidisciplinary colleagues to ensure coordinated care
  • Support discharge planning and facilitate smooth transitions between services
  • Maintain accurate and timely clinical records in line with professional and organisational standards
  • Participate in clinical supervision, service development initiatives, audit and quality improvement activities
  • Delegate appropriate tasks to Rehabilitation and Therapy Support Workers
  • Attend team meetings, clinical forums and in-service training sessions
  • Contribute to the continuous improvement of patient outcomes and service delivery. We are looking for someone who is enthusiastic about rehabilitation and community-based care and is committed to delivering excellent patient experiences.
You will have: Essential
  • Degree or Diploma in Physiotherapy
  • HCPC registration as a Physiotherapist (or be awaiting registration as a final-year student)
  • Strong assessment, treatment and clinical reasoning skills
  • Excellent communication and interpersonal abilities
  • Effective organisational and prioritisation skills
  • Good IT skills, including Microsoft Office applications
  • Ability to work independently and as part of a multidisciplinary team
  • Full UK driving licence and access to a vehicle for work purposes
You will have: Desirable
  • Membership of the Chartered Society of Physiotherapy (CSP)
  • Previous experience in community, rehabilitation, neurology, musculoskeletal or orthopaedic settings
  • Understanding of clinical governance, audit and quality improvement
  • Evidence of continuing professional development, Degree or Diploma in Physiotherapy, HCPC registration as a Physiotherapist, strong assessment, treatment and clinical reasoning skills, excellent communication and interpersonal abilities, effective organisational and prioritisation skills, good IT skills, ability to work independently and as part of a multidisciplinary team, full UK driving licence and access to a vehicle for work purposes.
