Job summary

Hywel Dda University Health Board is seeking an experienced Occupational Therapist to join our Community Integrated Stroke Team in Ceredigion, delivering home-based rehabilitation to support return to independence after stroke.

You will work autonomously within a multi-professional team, delivering assessments, developing person-centred plans, and evaluating outcomes while supporting learning and service development.

Qualifications

• Degree in Occupational Therapy and HCPC registration are required.
• Significant postgraduate experience as an Occupational Therapist across settings is expected.
• Experience working in community settings and with groups is beneficial.

Responsibilities

• Autonomously manage an Occupational Therapy caseload with complex presentations.
• Collaborate with a multidisciplinary team to deliver rehabilitation and reablement.
• Delegate and supervise staff as required.
• Travel within the County for home visits; mileage allowance and lease car scheme available.
