Qualifications

• Evidence of AGORED Level 3 qualification or equivalent
• Willingness to complete AGORED Level 4 within timescale
• Numerate and literate to GCSE Grade C or above
• IT skills with basic knowledge of Microsoft Office
• Advanced IT skills
• Evidence of role-related CPD activities
• Knowledge of Health Board priorities and practices
• Knowledge of rehabilitation interventions across therapies

Responsibilities

• Promote a person-centred approach to rehabilitation.
• Deliver and adapt specialist rehabilitation programmes in community, clinic and home settings.
• Monitor progress and provide feedback to the multidisciplinary team.
• Provide education, advice and practical strategies to patients and carers.
• Communicate effectively with patients, families and professionals.
• Maintain clinical records and administrative tasks.
• Contribute to service development, audits and governance.
• Support students, support workers and new staff through supervision and training.
• Participate in supervision and professional development including AGORED Level 4 where required.
