Job summary

Hertfordshire Community NHS Trust is seeking a Band 6 Occupational Therapist to join our stroke rehabilitation team. You will work with a dynamic multidisciplinary team to support people after stroke, helping patients regain independence and achieve personal goals in the community.

In this role you will conduct neurological assessments, develop person-centred treatment plans, supervise junior staff, contribute to service improvement, and participate in weekend rotas.

Qualifications

• Must be registered Occupational Therapist (OT) with relevant NHS banded experience.
• Experience in neurological rehabilitation and stroke recovery is desirable.
• Ability to work autonomously within a multidisciplinary team.

Responsibilities

• Provide specialist, personalised occupational therapy rehabilitation in patients’ homes and community settings.
• Manage your own caseload as an autonomous practitioner with supervision and senior OT support.
• Complete comprehensive neurological assessments and develop meaningful, person-centred goals.
• Deliver and adapt evidence-based treatment plans using recognised assessment tools.
• Coordinate care with patients, families, carers and the MDT, and support discharge planning.
• Assess and manage clinical risk with clear, timely documentation.
• Support and mentor Band 5 OTs, rehabilitation assistants and OT students.
• Contribute to training, audit, service improvement and evidence-based practice.
• Participate in a weekend working rota.
