Job summary

Hywel Dda University Health Board is seeking an experienced Occupational Therapist to join our Community Integrated Stroke Team in Ceredigion, delivering home-based rehabilitation and supporting patients to return home from acute units.

You will work within a multi-professional team, provide person-centred care, travel within the county, and participate in service improvement with supervision and flexible working arrangements.

Qualifications

• Degree in Occupational Therapy and HCPC registration.
• Experience as OT in community settings.
• Ability to travel within the County and surrounding areas.

Responsibilities

• Autonomously manage an occupational therapy caseload including complex presentations.
• Work within a multi-professional team to deliver rehabilitation and reablement.
• Collaborate with health, local authorities and third sector for best outcomes; support supervision and development.
• Delegate and supervise OT staff, students and volunteers as required.
