St Luke’s Hospice seeks a compassionate Community Nurse to deliver high-quality palliative care to patients in their own homes, joining a multidisciplinary team and providing holistic physical, psychological and emotional support with dignity and respect.

You will assess patients, support families, attend MDT meetings, administer medications safely, and mentor junior staff while promoting evidence-based practice in a busy community setting.

Competenze

• Registered General Nurse (RGN) with substantial palliative care experience.
• Experience working as part of a nursing team in a community setting.
• Knowledge of the palliative care approach and end-of-life care.

Mansioni

• Undertake comprehensive and holistic nursing assessments.
• Provide support and guidance to patients, families, and colleagues.
• Work collaboratively with the multi-disciplinary team, attending MDT meetings.
• Administer medication safely and in line with NMC and hospice guidelines.
• Demonstrate leadership by supervising and mentoring junior staff.
• Contribute to audits, research, and service development.
