Job summary

St Luke's Hospice Plymouth is seeking a motivated individual to join its End-of-Life Urgent Care Team. You will support patients at the end of life in their own homes or preferred place of care, helping them remain where they feel most comfortable and avoid unnecessary hospital admissions.

Responsibilities

• Provide end of life care to patients in the specialist unit or in the community to meet the patient’s preferred place of care/death.
• Work as part of the multidisciplinary team to provide individualised care for patients and their families.
• Support patients and their families at an important time in their lives.
