Meeveem Limited invites an enthusiastic Advanced Care Practitioner to join a patient-centred, forward-thinking team based in Louth. You will work autonomously to assess and treat patients across all ages, delivering high-quality clinical care in the practice and, where required, in patients' homes.

Qualifications

• Registered Nurse / Paramedic / Allied Health Professional registration is required.
• Strong clinical assessment and decision-making skills are essential.
• Ability to work autonomously and as part of a team.
• Excellent communication and organisational abilities.
• Confident in seeing patients of all ages.

Responsibilities

• Provide high-quality clinical assessment, diagnosis and treatment.
• Manage a caseload of patients independently.
• Conduct a mix of face-to-face and telephone consultations.
• Undertake home visits where clinically necessary.
• Work collaboratively with the general practice team to meet patient needs.
• Maintain accurate clinical records using SystmOne.
• Support delivery of policy and procedures and provide nurse leadership as required.
• Contribute to practice initiatives and continuous improvement.
