Job summary

Aylesford Medical Centre, based in the Royal British Legion Village, is recruiting a part-time Advanced Nurse Practitioner to deliver advanced nursing care by phone and face-to-face. The post includes diagnosing and managing acute and chronic conditions, prescribing and occasional on-call and home visits.

Applicants need a non-medical prescribing qualification, experience with EMIS and Docman, and registration as an advanced nurse with MSc or equivalent; a special interest in women's health is

Qualifications

• Registered first level nurse with an MSc or equivalent.
• Independent prescribing qualification required.
• Long-term conditions qualification required.

Responsibilities

• Deliver advanced-level clinical nursing care, assessing, diagnosing and managing acute and chronic conditions.
• Consult with patients by telephone and face to face, with on-call duties and occasional home visits.
• Prescribe and review medication within scope of practice and evidence-based protocols.
• Request and interpret diagnostic tests such as X-rays, blood tests and scans.
• Refer patients to the multidisciplinary team, secondary care and other agencies.
• Carry out clinical procedures and services such as blood pressure checks, contraception, immunisations, dressings and suture removal.
• Manage health screening programmes and contribute to QOF and practice clinical targets.
• Keep accurate consultation records and clinical systems up to date, and make item of service claims.
• Act as clinical leader, mentor junior staff and support staff development.
• Develop practice policies and protocols with the Practice Manager, support infection control and carry out agreed audits.
• Maintain own revalidation, personal development plan and regular clinical supervision.
• Communicate sensitive information to patients and carers and involve them in care planning.
