One Health Lewisham Ltd invites applications for an Advanced Clinical Practitioner to join our growing community healthcare team in Lewisham. You will assess, diagnose and manage patients with complex conditions, delivering evidence-based care across a range of primary care services.

Qualifications

• Registered healthcare professional with current UK registration (NMC or relevant body).
• Masters degree in Advanced Clinical Practice or equivalent.
• Independent Non-Medical Prescriber (where applicable).
• Minimum one year's experience in General Practice.
• Experience using EMIS clinical systems.

Responsibilities

• Independently assess, diagnose and manage patients with complex conditions.
• Develop evidence-based treatment and management plans.
• Request and interpret investigations.
• Prescribe medications where qualified.
• Deliver holistic, patient-centred care across community settings.
• Collaborate with GPs and multidisciplinary teams.
• Provide clinical leadership and mentorship.
