Job summary
Buckinghamshire Healthcare NHS Trust is seeking a motivated Qualified District Nurse to join our Community High Intensity User Service as a Nurse Case Manager. You will lead holistic assessments and develop personalised care plans for high intensity users across countywide services.

- Qualified District Nurse with authority to manage complex patient needs.
- Experience coordinating multi-disciplinary teams in community settings.
- Ability to develop personalised care plans for high-intensity users.
Responsibilities
- Undertake comprehensive holistic assessments for high-intensity users.
- Develop personalised care plans addressing complex health and social needs.
- Lead coordination for multi-agency input and ensure effective communication across services.
- Promote self-management and advance care planning with patients and carers.
- Monitor patients to prevent deterioration and reduce avoidable admissions.
- Contribute to service development and quality improvement.
