Job summary

Bucks Healthcare NHS Trust is seeking a part-time District Nurse / Community Case Manager to join the Amersham Adult Community Healthcare Team. You will deliver nursing interventions at patients' homes, requiring car access and a broad NHS skill set including IV care, catheter management, palliative and end-of-life support, wound care and complex care planning.

The role supports multidisciplinary teams, with induction and ongoing skill development.

Qualifications

• Must have a valid nursing qualification and current registration.
• Wide range of NHS clinical experience preferred.
• Proven ability to manage caseloads with high-risk patients.

Responsibilities

• Provide nursing interventions in patients' home environment.
• Implement case management for individuals at risk of hospital admission.
• Assess physical and psycho-social needs and deliver treatment based on best available evidence.
• Improve health outcomes using advanced nursing skills.
• Manage referrals to develop community care programmes.
• Support long-term condition management within the community team.
