Buckinghamshire Healthcare NHS Trust is recruiting a Community High Intensity Nurse Case Manager in Aylesbury. You will manage a caseload of high‑intensity users, develop personalised care plans and coordinate multi‑agency input across community, primary care and acute services.
The role emphasizes proactive case management, self‑management support, and reducing avoidable healthcare use through coordinated care and crisis planning.
Qualifications
- Registered Adult Nurse with current NMC registration.
- Community Specialist Practitioner qualification or equivalent.
- Demonstrated continuing professional development and CPD activity.
- Full UK driving licence and access to a vehicle.
- Master's degree or equivalent experience is desirable.
Responsabilités
- Manage a designated caseload of high-intensity users with holistic assessments.
- Develop personalised care plans addressing complex health and social needs.
- Lead multi-agency input and coordinate primary care, community and hospital services.
- Monitor patients at risk, intervene after ED attendances or discharges, and lead reviews.
