Job description

To practice autonomously and manage a caseload of patients primarily in community settings including nursing homes, residential homes and patients homes. Working closely with other members of the MDT Regular general outpatient clinics, and regular inpatient cover providing a an inpatient service to Alton Community Hospital, including to patients who are not necessarily receiving nutritional support.
