Sandwell & West Birmingham NHS Trust is seeking a Band 6 nurse to join the Community Heart Failure Team. The successful candidate will treat patients at home or in clinic settings, coordinate holistic care, and support junior staff within a collaborative team.
You should have heart failure experience and be eligible for NMC registration. This full-time role offers opportunities to specialise within a specialist heart failure team, with a focus on improving life for patients in the community.
