Job summary

Sandwell and West Birmingham NHS Trust is recruiting a Band 6 nurse to join the Heart Failure Team in West Bromwich. The role involves managing heart failure patients at home or in clinics, with a focus on holistic nursing and care coordination.

You will work within a supportive team and participate in an IV furosemide pathway. The post requires nursing qualification at diploma/degree level, with possible V300 non-medical prescribing and ALS/ILS development.

Qualifications

• Experience in management of patients with heart failure per NICE guidelines.
• Experience in clinical assessment and auscultation skills on heart failure patients.
• Experience in initiation, titration and management of heart failure medication.
• Experience of autonomous practitioner.
• Experience of managing heart failure.
• Experience of working individually as well as part of a team.
• Experience of participating in clinical audit.

Responsibilities

• Treat patients in their own home or clinic setting with heart failure.
• Coordinate and provide skilled holistic nursing for heart failure patients.
• Assess, plan, implement and evaluate individual care plans.
• Adhere to Trust and National Service Framework and NICE Guidelines for Heart Failure.
• Provide admission avoidance management and facilitated discharges.
• Be part of the IV furosemide pathway.
• Support junior staff within the team.
• Provide ongoing physical, psychological support and counselling to patients and carers.
• Deliver care within secondary care setting and liaise with acute heart failure teams across care levels.
