Hertfordshire Community NHS Trust is inviting applications for a nurse to join the Integrated Heart Failure Service (IHFS) in the community, developing specialist knowledge and skills.
The post targets experienced band 5 or band 6 nurses, working with Clinical Lead and CN Specialists to assess, plan and implement care, including heart failure medication titration and remote monitoring. The service operates five days per week and emphasizes teamwork and patient dignity.
This post offers the successful candidate the opportunity to develop the specialist knowledge and skills required to support heart failure patients within the Integrated Heart Failure Service (IHFS). The post holder should be an experienced band 5 or band 6 nurse looking to develop their own knowledge and skills within the heart failure specialty, working with the Clinical Lead and the Clinical Nurse Specialists within the IHFS to assess, plan, and implement specialist nursing care to patients in the community with heart failure. The core elements of the IHFS service include heart failure medication titration, admission prevention and supportive discharge from hospital. The successful candidate will be committed to working as part of a team, have good communication and decision‑making skills. HCT will prioritise recruitment of workers living in England under the new immigration rules for care workers effective from 9 April 2025. Employers must first prove that they have attempted to recruit a worker from within England who needs new sponsorship. This ensures that those who came to the UK to pursue a career in adult social care can do so.
