University Hospitals Coventry and Warwickshire NHS Trust in Coventry is recruiting an experienced Heart Failure Specialist Nurse to deliver high-quality, patient-centred care across acute, community, virtual and home-based settings.
You will provide expert assessment and management for people living with heart failure, work with the MDT to optimise therapy and lead the Heart Failure Nursing Team, developing evidence-based plans.
An exciting opportunity has arisen for an experienced and motivated Heart Failure Specialist Nurse to join the Coventry and Rugby Heart Failure Team.
We are looking for a skilled clinician who is passionate about delivering high‑quality, patient‑centred care and improving outcomes for people living with heart failure. This role offers the opportunity to work across acute, community, virtual and home‑based settings while developing advanced specialist knowledge and clinical leadership skills.
As a Heart Failure Specialist Nurse, you will provide expert assessment, management and support for patients with heart failure. Working autonomously and as part of a multidisciplinary team, you will deliver evidence‑based care and contribute to the ongoing development of the service.
You will review patients through outpatient clinics, virtual consultations, telephone reviews and home visits, ensuring patients receive timely, personalised care throughout their heart failure journey.
You will be joining a friendly, supportive and well‑established specialist team committed to delivering innovative heart failure services across Coventry and Rugby.

You will review patients through face‑to‑face and virtual clinics, Hospital at Home, home visits and telephone consultations.
You will work closely with the multidisciplinary team (MDT), GPs, healthcare professionals, patients and carers to ensure safe, effective and personalised care.
