South Bristol Rehabilitation units are located within South Bristol Community Hospital and have built a reputation for delivering excellent patient-centred care and enthusiastic teamwork.
As a Healthcare Support Worker, you will assist with the direct care of patients delegated to and supervised by a registered nurse.
Your role will support the nursing team, work collaboratively with the multidisciplinary team, and contribute positively to the delivery of high-quality, compassionate care centered around recovery and stroke rehabilitation, respecting the patients' wishes.
The service supports patients to recover and regain independence after admission to an acute hospital. The team includes registered nurses, therapists, advanced clinical practitioners, General Practitioners, and unregistered support workers.

To assist registered nurses and healthcare professionals in delivering individualized holistic care with compassion, dignity, and respect.
Work with the multidisciplinary team and contribute to harm prevention by participating in risk assessments such as nutrition, pressure area care, continence, and falls.
Assist patients with their care and nutritional needs.
Safely and accurately record physiological observations like blood pressure, pulse, respirations, and temperature, and escalate concerns to the registered nurse responsible for the patient’s care.
Report relevant patient care information back to the registered nurse.
Assist patients in mobilizing safely using prescribed equipment and techniques, and promote independence in activities like washing and dressing.
Support prescribed rehabilitation and therapy programs under supervision, facilitating safe and timely discharges that reflect individual choices and maximize independence.
