University Hospitals Coventry and Warwickshire NHS Trust is seeking an experienced Health Care Support Worker (Band 3) to join the Coventry Community Heart Failure Team. You will support heart failure patients across community, clinics and home settings, working with specialist nurses and consultants to deliver safe, high-quality care.
In this role you will contribute to patient assessments and ongoing care, help families manage conditions, and develop clinical skills through training.
We are seeking an experienced and compassionate Health Care Support Worker (Band 3) to join the Coventry Community Heart Failure Team. Working alongside heart failure specialist nurses, consultants and the wider multidisciplinary team, you will support the delivery of safe, high-quality and patient-centred care to people living with heart failure and other long-term conditions.
The role involves working across community, clinics and virtual settings, including patients' homes, with opportunities to develop your clinical skills and knowledge. You will contribute to patient assessments, ongoing care and support, helping patients and their families to manage their condition and improve their wellbeing.
We are looking for a motivated team player with excellent communication skills, a compassionate approach and experience within a healthcare setting. An NVQ Level 3 in Health and Social Care or equivalent is essential, with phlebotomy, ECG and heart failure experience desirable.
