Advanced Nurse Practitioner with a wide cardiology experience, and specifically within the subspecialty of Heart Failure to work as part of the Heart Failure Team here at Blackpool Teaching Hospitals. We are part of the Tertiary Cardiology service and have an excellent multidisciplinary team made up of Heart Failure Consultants, Heart Failure Specialist Nurses (both acute and community), Cardiology Device Nurses, Occupational Therapist and support staff. You will have use your specialist knowledge and skills to independently provide excellent care to our Heart Failure patients. You will be expected to work autonomously, and non-medical prescribing is fundamental in supporting this role.
The role will deliver the 4 pillars of advanced practice delivering clinical practice, leadership and management, education, and research and support with training and development of junior staff.
You will be expected to work closely and be supported by the Heart Failure Service leads providing a proactive role in quality, service and system improvement and will assist in the monitoring and auditing of clinical standards of care within the defined area.
The post holder will assess patients with undifferentiated and undiagnosed problems, evaluating their health care needs, based on highly developed nursing/AHP knowledge and skills.
Screen patients for disease factors and early signs of illness. Make differential diagnoses using decision-making and problem-solving skills.
The role will include the initial triage, diagnosis, utilising advanced clinical decision making and management of Heart Failure to support our patient cohort, both in the acute and community setting.
With the clinical team, l eading on service improvement work including review heart failure diagnosis pathways in line with best practice.
Have a supportive role in helping people to manage and live with heart failure.
Have the authority to admit or discharge patients from their caseload and refer patients to other health care providers as appropriate.
Work collaboratively with other health care professionals and disciplines.
Provide a leadership and consultancy function as required.

If the role you have applied for requires a Disclosure and Barring Services (DBS) check we will administer this as part of your pre‑employment checks. Please note, you will be required to repay the cost on appointment. This will be collected via a salary deduction. You can choose whether to pay this over 1-3 months from your salary or as a one‑off payment on commencement in post. The level of check required depends on the role that you have been offered. Currently the charges are - Basic DBS check £24, Standard DBS check £24 and Enhanced DBS check £45.
You are encouraged where possible, to register for the DBS update service. This is an annual registration fee of £15. By registering for the update service you will not have the additional cost of repeated disclosures.
Should you withdraw your application, you may be required to reimburse the cost of the DBS check.
DBS checks remain free of charge for volunteer positions.
