This role offers the chance to work within a bedded rehabilitation unit, supporting adults with a range of physical disabilities and functional impairments resulting from neurological conditions. You will manage patients and assist qualified staff in delivering individual and group treatment programmes within an agreed scope of practice. Working as part of a dedicated multidisciplinary team, you will contribute to nursing interventions and ensure clear handover of patient progress and any concerns to relevant ward or care home staff.

The role also includes supporting discharge processes, such as assisting with nursing discharge documentation and referral forms as well as managing stock take processes to ensure accurate inventory control.

We are looking for a motivated and compassionate individual who is committed to supporting patients to achieve their goals. You will be able to work independently when required, demonstrate excellent communication, organisation, and problem-solving skills, and manage your time effectively.

Successful applicants are expected to present clear and relevant evidence of the competencies and responsibilities detailed in the attached Job Description and Person Specification, together with a demonstrated commitment to the Trust's values of Accountability, Inclusion, Compassion, and Empowerment.
