Job summary

HCRG Care Group’s Livingstone Community Hospital, Dartford, is seeking a Healthcare Assistant to join our Community Nursing Team. You’ll deliver compassionate, person-centred care to patients at home and in community clinics across Dartford and North Kent.

Working with Community Nurses, you’ll support wound care, diabetes management and palliative care, document care electronically, and help prevent unnecessary hospital admissions while accessing ongoing development through our Learning

Qualifications

• NVQ Level 3 in Health & Social Care, Care Certificate or equivalent qualification.
• Experience in healthcare, community or care setting.
• Strong communication and IT literacy with MS Office applications.

Responsibilities

• Deliver high-quality care to patients in their homes and community clinics.
• Support wound care, diabetes management and end-of-life care.
• Undertake delegated clinical activities within your scope.

About the Role

Based at Livingstone Community Hospital, Dartford, you'll work as part of our Community Nursing Team, supporting patients to receive high-quality care in their own homes and community clinics.

This is a varied and rewarding role where you'll work alongside Community Nurses to provide compassionate, person-centred care that promotes independence, wellbeing and dignity.

Our teams are equipped with modern technology, enabling you to access patient records and complete documentation remotely, helping you spend more time delivering care and less time travelling back to base.

Located just off the M25, Livingstone Community Hospital benefits from excellent transport links and is within easy reach of Bluewater Shopping Centre and local amenities.

Main Responsibilities

Working closely with Community Nurses and the wider multidisciplinary team, you will:

• Deliver high-quality care to patients within their own homes and community clinic settings
• Support patients with a range of healthcare needs, including wound care, diabetes management and end-of-life care
• Undertake delegated clinical activities within your scope of competence
• Monitor and report changes in patients' conditions to registered clinicians
• Promote patient dignity, independence and wellbeing at all times
• Accurately document care provided using electronic patient record systems
• Develop positive working relationships with patients, families and healthcare professionals
• Contribute to preventing unnecessary hospital admissions through effective community-based care
• Support the continual improvement of services through shared learning and best practice

About You

We're looking for a compassionate, motivated and organized individual who is passionate about delivering outstanding patient care.

To be successful in this role, you'll have:

• An NVQ Level 3 in Health & Social Care, Care Certificate plus relevant experience, or equivalent healthcare qualification and experience
• Previous experience working in a healthcare, community or care setting
• Strong communication and interpersonal skills
• Excellent organizational skills and the ability to manage a varied workload
• Confidence using IT systems, including Microsoft Outlook, Word and Excel
• The ability to work independently and as part of a multidisciplinary team
• Full UK driving license
• Access to a vehicle for work purposes
• Ability to travel across the service area in a timely manner
