Job summary

Forward Trust is recruiting a Desistance and Wellbeing Practitioner (More Than My Past Practitioner) for the Connections Team at HMP Millsike in York. This is a full-time, on-site role delivering one-to-one support, group interventions and wellbeing workshops to help prisoners improve physical, mental and emotional wellbeing.

You will work with prison staff, healthcare professionals and external partners to promote recovery-focused living and support prisoners on their path to healthier

• Coaching and mentoring service users to achieve positive, lasting change
• Supporting and developing prisoner Health & Wellbeing Champions
• Promoting healthy lifestyles, recovery and wellbeing across the prison
• Organising health promotion events and awareness campaigns
• Building strong partnerships with healthcare, mental health and substance misuse services
• Supporting the delivery of a varied programme of wellbeing activities, including evenings and weekends
• Maintaining accurate records, care plans and case management documentation

Desistance and Wellbeing Practitioner (More Than My Past Practitioner) - HMP Millsike

We're looking for a passionate and motivated More Than My Past Practitioner (Desistance and Wellbeing Practitioner) to join Forward Trust's Connections Team at HMP Millsike. More than My Past is a programme that we are delivering where we offer one-to-one support and workshops that promote personal development and rehabilitation, helping people build a life beyond crime.

This is a full-time position working 35 hours per week, offering a unique opportunity to support people in custody to improve their physical, mental and emotional wellbeing, build resilience, and develop healthier lifestyles that can be sustained on release.

Working as part of our innovative Health & Wellbeing service, you'll collaborate with prison staff, healthcare professionals, peer mentors and external partners to create a positive, recovery-focused environment across the prison.

What You'll Be Doing
