Job Summary
The Health and Wellbeing Coach will deliver group and one‑to‑one sessions on healthy eating, physical activity and health and well‑being to enable patients to meet personalised care plans. The coach will engage patients and use health‑coaching techniques to support them in taking an active role in their own health. The Social Prescribing element of the role will support people to strengthen community resilience, reduce health inequalities and tackle wider determinants of health such as debt, poor housing and physical inactivity.

Key Responsibilities
- Identify patients with long‑term conditions and assess their knowledge, skills and confidence.
- Provide clinical or non‑clinical support to a cohort of patients, acting as a single point of contact to simplify access and coordination of services.
- Teach and support patients/carers to manage their own conditions and maintain independence through health‑coaching techniques.
- Develop personalised care plans, liaising with the practice, patient/carer and complex care teams.
- Proactively support practice targets for the number of patients seen by a health coach and engage patients on a regular basis.
- Participate in multidisciplinary team meetings, presenting patient progress and setting goals.
- Map and connect community activities and resources, building a local network of community health champions.
- Deliver community‑based public health initiatives such as physical activity, healthy eating and social connectedness.
- Maintain relationships with voluntary and statutory partners and promote the service within the Primary Care Network.
- Support the delivery of self‑care plans for conditions such as COPD, diabetes, asthma and multi‑long‑term conditions.
- Refer patients to other health professionals or agencies when appropriate.
- Identify and address barriers to accessing community support, using this insight to develop community‑based interventions.
- Promote the service to clinicians and users, building positive working relationships.
- Organise and contribute to awareness‑raising events that support people’s health and wellbeing.
- Communicate effectively with colleagues, patients and carers to meet patient needs.
- Play a key role in raising local populations’ awareness of support, groups and opportunities to achieve health and wellbeing goals.
Qualifications
- Educated to GCSE or equivalent.
- Commitment to professional and personal development.
- Mandatory accredited training for Health Coaching and Social Prescribing, including motivational coaching, behavioural change and goal‑setting.
- Experience in health promotion.
Desirable
- NVQ 3 or equivalent and/or relevant basic/first‑level professional qualification or working towards this.
- Experience in community development, adult health and social care, learning support or public health/health improvement (including unpaid work).
- Experience supporting people, their families and carers in a related role.
- Experience supporting people with mental health in any capacity.
- Experience with data collection and measuring impact of services.
- Experience in partnership/collaborative working across voluntary sector.
- Experience using clinical systems (e.g. SystmOne).
Knowledge and Skills
- Knowledge of personalised care approaches.
- Understanding of determinants of health: social, economic, environmental factors.
- Knowledge of the needs of patients with long‑term conditions.
- Commitment to equality, diversity and inclusion.
- IT skills: word processing, email and internet.
Desirable Knowledge
- Knowledge of local public health issues.
- Experience training volunteers.
- Knowledge of community development approaches.
